Lots of good songs mentioned so far.
Johnny Cash - Hurt is still probably my favorite. I am also very partial to 311 - Love Song. It's better than the original.
I will add another that I really like.
Pantera - Planet Caravan
I am not even a Pantera fan but they nailed this cover of a Sabbath song.

Not sure if this has been mentioned yet, but I dig a lot of what Me First and the Gimme Gimmes have released. They punkify a lot of different types of music - country, showtunes, classic rock, etc.

Brown Eyed Girl by Jimmy Buffet (Originally Van Morrison) - I swear half the people I know don't know this was originally by The Man.
Wonderwall by Ryan Adams (Originally by Oasis) - One of the best covers ever in my opinion.
Somewhere Over The Rainbow/What A Wonderful World Medley by Israel Kamakawiwo'ole (Originally recorded by Louie Armstrong and Judy Garland) - Amazing piece of music.
A Pirate Looks At Forty by Jack Johnson (Originally by Jimmy Buffet)

Not a big fan but that could be because there aren't a whole lot of covers I do like. I like the original feel to songs because while listening to covers, I try to make it sound like the original if that makes sense.

My favorite: "I Will Survive" by Cake. They just make this work in their own way. The bass line is great.
I also enjoy STP's version of "Dancing Days", but that is just because I am an STP homer.
Least Favorite(s): "Bad Company" by Five-Finger Death Punch. They took a beautifully subdued song and turned it into a growly hard emo-rock mess.
I have also stumbled upon 30 Seconds to Mars cover of Lady Gaga's "Bad Romance". I think she is the best pop artist out there and would consider myself a fan, but 30 Seconds to Mars just butchered the song as only Jared Leto and his over-dramatic and over-produced band could do. Velvet Underground's "Venus in Furs" covered by Devotchka.
