Mine happened a little over a month ago (however, I was extremely hung over). We had a management outing for work at a local park where we were playing several games during the downtime around lunch. A bunch of us got together to play kickball on the nearby baseball field with a ball we found left at the park. We probably had around 15 people, 10 or so girls. The guys playing were all directors in our company, and I would say not the most athletic group but decent, so I was trying to impress.
First time I get up to bat, I run up to kill this slow rolling son of a pitch, swing my foot for the fences and completely miss, nearly spining circles and falling to the ground. Everyone had a good laugh. Although my 2nd swing I did hit an inside the parker, but then missed a few easy catches in the outfield later as well.
Wiffing a kickball pitch at 28 = megafail.

Middle School football fail. When I entered the 7th grade I was at a new school due to moving. I got the starting QB position and at our first game we got the ball first and we got a first down. The parents started yelling and cheering their hearts out. They were going a little overboard for just a first down and my Dad was in the stand and asked the other parents why they were do excited about a first down. One parent responded that is has been 4 years since the football team has had a first down!

Another middle school track one. The shuttle hurdle relay team I was once finished 2nd, out of 2 teams. It was our best finish.

My senior year and it was senior night. Just about everyone important in my life was at the game which meant a lot. I wasn't the most talented guy on the team (we have a couple college players,) but I was by far the most clutch. I had multiple game winning hits and game-saving catches my senior year. Radio guys loved me.
Anyhow, it's the bottom of the 7th with no outs (last inning before extras) and I lead of with a perfectly hit double, right down the left field line to the fence. Next guy grounds out to third, so I couldn't advance. I noticed the pitcher hadn't checked on me the entire at-bat, so I figured I would widen my lead.
Got picked off. We lose in the 9th.

Disclaimer: my main sport was wrestling.
Freshman in HS, played on the football team, only to fill bodies in on special teams. On the kickoff team, ran in to gang tackle the returner, and in my infinite brilliance I took a shot at knocking the ball out. I was appalled when I was flagged for a personal foul. The video shows me winding up and punching the guy in the ribs.

Got another one. I was playing baseball at Boone, hit one to deep left field, and immediately lost it in the sun. I had a hit a few HRs in one my high school career (humblebrag) and even though it was opposite field, I thought I may have gotten this one. As I rounded first I thought I saw the ump giving the HR signal. I was trotting around 2nd base by the time the shortstop told me I was out. It was terribly embarassing.

My very first tennis tournament I received a 4th place trophy, which was given to me by my parents on behalf of our coach as I didn't stick around for the end of the tourney. They acted all very proud of me. I didn't realize why in the hell I would get a trophy as I only played 2 matches and got murder both times, until a week later at practice I realized there were only 4 people in my bracket.
Not as pathetic as it sounds though as this was like 4th grade, and I had only picked up a racket and been to one lesson prior.

This is a great thread, I feel like there should be a poll at the end to pick a winner.
So far I'm liking the story of being so far behind in the 1 mile that you have to waive off the official to let them know you are a lap behind and not the winner.

We didn't have a 4x8 team cause not a lot of us ran the 800. Well, our coach found out that only 2 other schools entered a 4x8 team. I was one of the lucky ones chosen for that race. HATED the 800.
That bronze was my first medal, though!

During my 7th grade basketball season I recorded more fouls than points in each of our 5 games. I finished the season with a double digit foul total and 6 points. Needless to say, I decided to stick with wrestling from that year forward.
Additionally I was able to successfully complete less passes in a JV football game my Junior year than the total number of times I was sacked. I walked away from that game going 3-12 for 63 yds, 1 TD, 2 Fumbles Lost and was sacked 11 times. It did not help matters that the entire right side of our offesive line weighed a combined 300 lbs and our Center was morbidly obese, which seemed to make it difficult for him to stand up and actually block someone after he had snapped the ball. My grandchildren are going to be so proud of me someday when I am telling them these tales of greatness.

Not sure how I could forget the only wrestling tourney I ever was in. I had been doing the "monday night wrestling in the high school weightroom" for several years and in 6th grade decided to sign up for a tourney. First match, Probably less than 30 seconds in, I am on my back staring at the lights. End of match, however I didn't know that. I had only ever gone to practices. Didn't make it out of 1st period in either of my 3 matches that day. Got a 4th place trophy. Never wrestled again.
Exaggeration is a BILLION times worse than understating. -

Since he hasn't posted yet in this thread, I will go ahead and tell KFitzys most disappointing moment.
A little background info:
Senior year he led the team in points, rebounds, and steals. He had no stars on rivals, but that's because he told Yahoo! that starz don't matter. Otherwise, he would've been a top 50 recruit. UNC, Duke, Kansas, Drake, UNI, Nebraska, and Iowa State are all in attendance to watch Fitzy lead the team to state.
So the team is ranked #2 in state. Undefeated. Playing the #7 team in the substate finals.
They lose 58-12.
The reason? Fitzy decided just to shoot trick shots the entire game.
