Most used - my 12V cordless Milwaukee radio (the 12V drill I bought with it is 2nd most used)
Least used - slide hammer I bought to replace a bearing while replacing a clutch on my last vehicle. My chain whip I use to replace cassettes on my bicycles has also probably only been used once or twice.
Last year fathers day my kids got me a Rigid drill, impact driver combo. That impact driver is AWESOME. I showed my brother in law last week. He didn't understand why it was better than a regular cordless drill. I got out a green treated 4x4 and drove a deck screw all the way through it and out the other side. The best thing about the Rigid is that the batteries are lifetime warranty.
Most used is B&D cordless drill/driver.
The tool I use the least right now but is my best is my Dad's old DeWalt radial (or radio if you're reading an eastsider's craigslist ad) arm saw.
Most unusual would be an old pair of dehorners we used on bull calves when I was a kid on the farm.
Most used: 3/8 and 1/4 drive socket sets (1/4 set also includes a bit driver). Between the two, there isn't much I can't take apart.
Least used: Old HP function generator (electrical signals) I picked up on ebay a few years ago. I verified that it worked upon receipt, but have never used it for anything.
Most useful: Vice Grips.
Most obscure: Pneumatic pop rivet gun. Way way way easier/faster than a hand riveter if you need to put in a bunch of pop rivets. We used it extensively building our photo booths.
Most used is probably my cordless screwdriver. My Honey-Do list always seems to be putting up a new ceiling fan, picture or hanging curtains. Least used is a pipe wrench -- which is good, because I hate plumbing.
