I like ameritrade. Good tools, fast trades, and they are pretty cheap. Never paid them a cent in account fees, just trade fees, and I do not trade much at all.

I use Charles Schwab. The fees are 12.95/trade (which is higher than most online brokerages) but they were about the only ones the reinvested dividends without a fee or even reinvested dividends. I have been extremely pleased with their service when I have called. They have answered a lot of questions for me.
I'm a big fan of Scottrade. I didn't do so well on my first few investments, and have yet to re-fund the account to a larger amount, but I've used them for two years and have yet to get a non-trade related fee. I also get interest on my balance.

E*trade sent me a letter a couple of months ago indicating that they would be begin charging maintenance fees for accounts that did not make a certain number of trades and that did not maintain certain minimum balance. From what I can tell, I meet the requirements, and haven't been charged anything...yet...
