Also, just caught this while I was re-watching it...
While Desmond is about to get his MRI the doctor says: "And you need the button. A panic button. If you need to stop, press it, but try not to because we'll have to start all over again from the beginning."
Desmond's job on the island was to push a button. He missed one, panicked, and had to turn the key, which resulted in the 'starting all over again from the beginning' like we are seeing in the alternate timeline.

One of the best episodes of the series: Desmond is still Charlie's caretaker in the sideways timeline, except that in this timeline Charlie keeps trying to kill himself and can't do it whereas on the island Desmond had to help Charlie avoid death around every corner. My favorite cameo was Desmond's driver played by Academy Award winner Fisher Stevens - whenever I see that guy it makes me think of Short Circuit. I think I may have to take back what I said about Juliet not returning to the show, now that both Charlie and Penny made an appearance this episode and they both star in FlashForward. Now that Desmond has a purpose that makes 4 groups that the Losties can choose to join: Desmond (Just him), MiB (Sayid, Claire, Sawyer, Kate), Jacob/Richard (Hurley, Jack, Sun, Ben, Illana) and Whidmore (Jin) - these next episodes are going to be great!

Some of the coolest episodes were when Desmond came to terms with having to let Charlie go. It still burdened him, but he stopped trying to avoid it.

Someone is going to have to control the MIB and keep evil under control, otherwise he will destroy the world. Controlling the MIB, the way it is done, who made the rules he and Jacob have to follow is the question. How the island falls into this as either the maker of the rules or is it a tool? The island is always referred to in the first person as a living being in and of itself. Is this all an allegory? etc etc etc

Glad to hear some more positive comments about the episode. Helps me to keep things in perspective. My gut reaction was that there was not enough "on island" development, so it is nice to see others' perspective on the alt-timeline, which has also had a quite a bit of development.

I think Desmond is Lost's version of Neo and the Matrix
We are having a discussion on my site and someone made a comment with regards to Jacob and MiB having done this many times, and Eloise and Whidmore having done this many times..
It made me think of the scene in the 2nd Matrix movie where Neo talks with the Architect, and is shown the multiple times it has played out...and that there is no choice to be made because you have already made a choice...it's over...its predetermined.
But the Neo we see in the Matrix movies is somehow different this time...and like the end of that series when Mr Smith says 'No, this can't be. You are supposed to die right here, right now...get away from me!'
I think Desmond is sort of this series' Neo, the last time around...this series began after the Matrix trilogy had finished..the Matrix was also loosely based on Christian theology, as is this series, plus other belief systems...

Just saw this, pretty interesting find...
In last week's episode Desmond was clearly NOT married. However, in the season premiere when he was on the plane speaking to Jack, he was wearing a wedding ring. The link below is to a couple screenshots of him with the wedding ring on just for proof. How is this possible? Are there two different Desmonds? Was the one on the plane "flashing" through time? Anyone else get the feeling that George, the limo driver, was pretty creepy too? Like borderline MIB-like?

And I am starting to think that the sideways timeline we are seeing is not any real thing at all...that it's sort of a Matrix world..that the MiB has created to trick people into thinking they have left the island when maybe they haven't at all
