Who's responsible for coaching the players to make those plays? And who's responsible for putting those players in the right position to make those plays?

Yes, ultimately it's on the players to make plays, but the coaches aren't even doing their part for that to happen. When it's the same thing year after year, it's a systematic problem. That's on coaching.
